We are looking for Band 5 Community Nurses to work within our Neighbourhood Teams based in Peterborough (at Botolph Bridge Health Centre and City Care Centre) for a fixed term of 12 months . There are currently 2 positions available. This post is eligible for a £1500 New Hire Bonus. An initial payment of £500 will be paid in the first month and a final payment of £1000 will be paid 1 year after your start date. New Hire Bonus will be pro rata for part time roles. The post involves working in an integrated way with housebound patients delivering care in a variety of domiciliary and care home settings. Our team consist of Occupational Therapists, Physiotherapists, Therapy Assistants, District Nurses and Mental Health Nurses You will be working as part of the Neighbourhood team which includes Community Nurses (RN), Advanced Clinical Nurse Practitioner, Physiotherapists, Occupational Therapists, Mental Health Nurses, other Mental Health Professionals and Team co-ordinators. You will link closely with colleagues outside of CPFT including General practitioners to support excellent nursing care at home, reducing the necessity for acute hospital admission. We welcome applications from Nurses with current community nursing skills, from newly qualified Nurses, Return to Practice Nurses and experienced Registered Nurses wanting to develop their skills further. We are looking for people who: Have experience of working in the Community Are educated to RGN level 2 Are able to demonstrate a commitment to high quality care and service provision Have the ability to work flexibly and as part of a team Have the ability to assess and formulate care plans Are able to teach and support student Nurses and Health Care Assistants Have enhanced communication and inter personal skills You will be responsible for: Keeping the patient at the centre of care, ensuring patient and carer participation in decision making Providing a high standard of care Providing a high standard of nursing intervention within a patient’s own home, with access to Specialist and Advanced Practitioners when required Working as part of a 24/7 service and a multidisciplinary team If this sounds like the role for you then please apply!
