An exciting opportunity has arisen within Wakefield Clinical Commissioning Group (CCG) to work within the continuing healthcare team undertaking comprehensive nursing assessments and determining the level of care required within the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care We are looking to appoint 1 RMHN and 1 RNLD to join the continuing health care team. The post holder should be a Registered Nurse ( RMHN RNLD) and have knowledge and experience of working with the National Framework for Continuing Healthcare and Funded Nursing Care (revised 2018) and any associated guidance and clinical tools The post holder will be responsible for completing assessments and reviews of individuals within care home settings and within their own homes. An ability to work collaboratively with a wide range of teams within Wakefield CCG and other partner organizations’ will be required to ensure the appropriateness of assessments and care plans e.g. provider services, acute hospital, hospice, Care Home Liaison Team, Social Services, TVN, Macmillan Nurse (care Homes) Community learning Disability Team The post holder will liaise with other agencies and refer as necessary if deemed appropriate, maintaining good relationships with colleagues.
