Learning disabilities services We provide crisis support and inpatient services and our community learning disability teams work in partnership with local councils to provide assessment and support for adults with learning disabilities. Social care We provide personalised social care support to people with a range of needs, including people with learning disabilities or mental illness, supporting people to live independently. LEARNING DISABILITIES SERVICE Specialist Health Practitioner - LD Enhanced Support Team For People with Learning Disabilities South Essex BAND 6 HOURS 37.5 PER WEEK This is an exciting time in Essex with the Learning Disability Service currently in a period of evolution that will focus on developing services that are open; compassionate and empowering and fit for the future. Transforming Care has come to an end but the future of how learning disability services are provided across Essex are continuing and will provide a county wide service known as Essex Learning Disability Partnership, a collaboration of the three agencies currently providing Learning Disability services – HPFT, EPUT and ACE. In South Essex we currently have two band 6 qualified Specialist Health Practitioner vacancies in our community learning disability service. The post holders will work with the Enhanced Support Team to provide support to those people with learning disability in the community with mental health or behaviour crisis in order to reduce the need for admission to hospital and; where admission is needed to work closely with the inpatient services and other providers to support timely discharge back into the community. You will also be part of an MDT working with those service users with more complex needs in the development of alternative approaches to managing behaviour including PBS. The post will require knowledge of Care and Treatment Reviews, Root Cause Analysis, Mental Health Act and Continuing Health Care. You will work directly with commissioners to make sure that risks are managed effectively and safely in the right place at the right time. You will also work alongside the Essex Community Offending Service (ECOS) and may be involved in repatriating service users who have been placed out of area and are returning to Essex. The posts will cover the South Essex area although there might be occasions when you are asked to travel to other areas within the service. You will need to be able to network and develop close working relationships with commissioners and other providers to demonstrate compliance with Key Performance Indicators and meeting service requirements. This may also include presenting to panel meetings to access funding. This service currently operates from 8am – 8pm Monday – Friday, although under the service transformation it is envisaged that the service will be extended to 9am – 5pm weekends and bank holidays.
