The Brighton and Hove Integrated Primary Care team is based at Brighton General Hospital and provides domiciliary nursing and therapy to the population cared for by Hove's Primary Care Networks. The Clinical Nurse Lead is an exciting and autonomous role supported by the Operational Team Lead and the Practice Development Nurses. It plays a key role in supporting staff to develop and maintain their clinical competency, ensuring teams deliver the right care at the right time to the individuals on the community nursing caseload. The successful candidate will hold a Community Specialist Practitioner or equivalent qualification. They should be able to demonstrate excellent communication and negotiation skills as well as a sound knowledge base linked to advanced physical assessment and treatment planning of individuals with complex comorbidities that require a multidisciplinary approach from the Primary Care Network. The Clinical Nurse Lead will be required to support the Community Nurses and the Primary Care Network partners to coordinate and deliver the care to this cohort of complex patients, ensuring that services wrap around and support them to maintain their health and wellbeing in their normal place of residence.
